- Act as an advocate for clients, ensuring that their best interests are top priority.
- Identify short and long term issues relating to general well-being during an initial evaluation.
- Create a customized care plan.
- Discuss difficult issues and help with resolution.
- Coordinate care and medical services including appointments and medication management.
- Evaluate, arrange for and monitor homecare services.
- Guide clients and families with appropriate facility placement when needed.
- Assist in times of crisis, helping families make difficult decisions.
- Provide referrals for legal, medical, or financial issues.
- Communicate with families on a routine basis.
- Act as liaison between families and medical professionals, answer questions and deal with emotional concerns as needed.
- Advocate for clients during the Community Medicaid evaluation process particularly at Independent Assessor and MLTC evaluations.
- Perform NYS PRI and Screens when needed.

Team
Sharon Mazzola is the third generation of registered nurses in her family. She has more than two decades of nursing experience after graduating from Boston College with a Bachelor of Science in Nursing. She has broad clinical experience, with a focus in critical care that included work in the Cardiac ICU at a leading NYC hospital. She has worked as a Care Manager with the geriatric population since 2018, and founded her own company, Kelly Care Management in January 2023. Sharon is a certified New York State PRI and Screen assessor and works as an advocate for people during the Community Medicaid screening process. Sharon works closely with her clients and their families to coordinate their care, advocate for their best interests, and enhance their quality of life.
Isabel Lawler joins the Kelly Care Management LLC team as the Director of Operations. With over 20+ years of experience in both the financial and operations sectors, Isabel will be handing the day-to-day operations of the company, ensuring continued success.
Prior to joining Kelly Care Management, Isabel served as Chief Financial and Operations officer in both the public and private sector. Her experience spans several industries with a concentrated focus in operations and efficiencies. Her tenure offers critical experience to navigate the operational strategies that will complement and enhance company growth.
Isabel holds a bachelor’s degree in Accounting from Manhattan College, has served on various boards and comes with an inherent knowledge of client services.
